All committee members must complete the following form to serve on a committee within the American Academy of Cosmetic Surgery (AACS).
By submitting this form, I am committing to:
The undersigned acknowledges that, as a condition of my participation on the noted Committee(s) of the American Academy of Cosmetic Surgery, any and all works that I create for, or are created by, the Committee(s), which may be copyrightable, shall be considered “works made for hire” as defined under the federal copyright law, 17 U.S.C. §101 et seq. To the extent that the works are inappropriate for treatment as “works made for hire,” the undersigned hereby grants and assigns to AACS all right, title, and interest, including copyright and all rights subsumed thereunder (including, but not limited to, the right to reproduce, prepare derivative works, distribute copies by sale or otherwise, perform, display and renew copyright), in and to such works. Thus, it is my express intent that AACS shall be the sole and exclusive copyright owner with respect to all works created by or for the abovementioned Committee(s).

- I will abide by AACS’s policies on conflicts of interest and will strive to avoid even the appearance of such conflicts.
